The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone, 250-586-0443, or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to either print and fill or on-line fill our three (3) new patient forms that are needed to enter you in our files. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. If you fill them out on line, when you click "Submit" they will be automatically emailed to the office. Thank you and please contact our office if you have any questions.
